Intake and Eligibility
Individuals interested in receiving services from the Washington County Board of DD must first complete the determination process. Eligibility requirements vary based on the applicant’s age.
Gather Documentation
Before reaching out, we recommend you have the required documentation on hand to ensure that the process is as easy as possible.
You will need:
A copy of your Social Security Card
A copy of your Birth Certificate
A copy of your Insurance Card
Proof of your disability (the most recent school, medical, and/or psychological records)
Guardians will need to provide Probate Court records showing proof of their appointment

Eligibility Requirements by Age
For ages 3 - 5
2 areas of developmental delay
A qualifying diagnosis as documented by a qualified professional
For ages 6 and up
A qualifying diagnosis documented by a qualified professional
Must be eligible based on the Children’s/Ohio Eligibility Determination Instrument (C/OEDI)
For individuals already receiving services, eligibility is re-determined at ages 3, 6, and 16

The intake process varies by age and specifics, which will be discussed during the initial request for services. For any age, we review documented diagnoses. For ages 6 and older, completion of the Children’s/Ohio Eligibility Determination Instrument is required.

The length of the process varies by case. If the WCBDD needs to request records from outside agencies, the process will take longer. Documentation submitted by individuals/guardians expedites this process. Examples of records include: psychological evaluations, medical records, hospital records, evaluation team reports (ETR), and Individualized Education Plans (IEP). Once supporting documentation is received, the C/OEDI will be scheduled.
